This is likely to be a traditional model.
To distinguish it from other boxes, I have called it the Blintz Box, but it very likely has various other names.

Steps: 18
Description: This is a square box with fairly high sides. If folded from two-sided paper, both sides will show in the finished model. More specifically, the sides and inner part of the box will be one color while the flat, outer bottom or top, depending on if you are folding the box or its lid, will be another color or pattern.
Two of the sides of this model are smooth while the remaining two are made up of a large, smooth side area flanked by two smaller triangles forming the corners of the box.
If turned upside-down, the blintz box has pockets or slots on all sides. It is a great option for a lid since it makes it possible to insert a decoration like the gift bow, Accessible Origami Volume 1, MDL0006.
This box is related to, and may be combined with the masu box, MDL0008 and the Crown Box, ADL0003 (both in Accessible Origami - Volume 1). Simply fold one masu and one blintz box or a crown box and a blintz box from squares that are the same size.
The boxes are fairly flexible so one could serve as a lid for the other.
If necessary, a slightly larger blintz box can be made as a lid by altering Step 13. Instead of folding in the sides all the way to center, leave a little space on both sides of the center line.
Remarks:
It is perfect to use for all occasions and it is fairly easy to fold.
Even though there are many steps, they are all fairly easy to complete and to memorize.
NOTE: This model starts from the blintz base. If you are able to achieve this without instructions, fold the blintz base once; then go to Step 5.
Step 1
Place a square piece of paper down on a hard, flat surface with its edges to the left and right, top and bottom.
Step 2
Fold the left edge over to meet the right edge. Crease and unfold.
Step 3
Fold the top edge down to meet the bottom edge. Crease and unfold.
Step 4
Use the crease lines on your paper to fold all four corners in to meet at the center of the square.
Step 5
Open your paper all the way again.
Step 6
Now turn your paper with the points facing up and down, left and right.
Step 7
Create a small triangle by folding the bottom point upwards to rest on the first fold line you encounter.
Crease well and leave folded.
Step 8
Repeat with the 3 remaining corners so you have a small triangle on each side.
Step 9
Refold all 4 sides of your model along the already folded crease lines, enclosing the triangles from the previous step.
Your model will now resemble a square picture frame.
Step 10
Turn over and position with edges left and right, top and bottom again.
Your model will have a smooth surface.
Step 11
Fold the left edge over to meet the right edge and crease lightly
Unfold again.
Step 12
Fold the top edge down to meet the bottom edge and crease lightly
Unfold again.
Step 13
Fold both the left and right edges to meet in the middle.
Crease well and leave folded.
You will now have a rectangle with four small triangles and a loose flap on each side of the vertical center.
Step 14
Fold the top layer of paper on the right over to the left.
Step 15
Fold the small triangles at the top and bottom over on themselves. Crease well and leave folded.
Step 16
Return the flap you folded over back towards the right.
Step 17
Repeat Steps 14 to 16 on the lefthand side of the model but in the opposite direction.
Step 18
Notice that there is a vertical slit at the center of your model.
Pick up the model, insert your thumbs into the vertical slit and pull to the sides, while gently pushing the top and bottom sides inwards into a square shape.
Carefully pinch all 4 corners into shape if necessary.
